NEW PATIENT MUST COMPLETE BOTH LINKS IN ORDER TO BE REGISTERED.
Once you click on the “Patient Portal” link, please go halfway down the page to where it says ‘New to Practice?” and click on “Pre Register.” Please fill in that information to the best of your ability.
Also please fill out the New Patient Paperwork through the link below.
These steps will ensure less time doing paperwork and more time with your MD on the day of your initial visit.
